Volleyball Intention Forms

Prior to being selected to our volleyball teams at Sherwood Heights Junior High School, students and parents must consent to the financial and time commitment associated with the team. Please fill in the appropriate form below - grade 8s: you will likely have to fill in both. 

Jr (Grade 7 and 8) Intention Form: **PLEASE NOTE - we do not currently have a coach for the Jr Boys team. If you are able to coach, please contact kristin.alexander@eips.ca  ; if we are unable to find a coach, we will be unable to run the team.
