Sherwood Heights Junior High

Registration booklets will be handed out to the Grade 6 students of our "feeder" schools just after spring break or can be picked up directly at Sherwood Heights or downloaded from our website (see below).

Students along with their parents complete the registration sheets selecting programs and options and numbering in order of preference.

The following forms need to be completed:

EIPS Student Registration Form

(only needed if your child is new to Elk Island Public School)

Grade Seven Registration Package

Interview/Photo Consent Form

Website Consent Form

Student Technology User Agreement Form

Academic Excellence Seven

(only needed if your child is applying for the Academic Excellence program) 

LOGOS Commitment form

(only needed if your child is applying for the LOGOS program)

Boundary Exemption/ Request to Change Schools

(only needed if this is not your boundaried school)

Student Record Release Form

(only needed if your child is coming from a school outside of Elk Island Public Schools)
